Addressing the clinical and non-clinical needs of individuals with complex conditions, Enhanced Care Management (ECM) is a statewide Medi-Cal benefit providing intensive coordination of healthcare and health services for qualified patients. Our ECM team meets beneficiaries wherever they are: on the street, in a shelter, in their doctor’s office, hospital, or at home.
